Pathways to Opportunities provides support for adults with learning disabilities, autism, complex needs and chaotic lifestyles. We provide a quality service for adults aged 18 + within the borough of Oldham.
We provide Care at Home, community, day service and respite support, working alongside individuals, their families and any involved professionals. We pride ourselves on our commitment to person-centred care, flexibility, high standards and bespoke packages of support. We have a well-trained and compassionate staff team, who deliver care in line with individuals’ care plans and risk assessments, ensuring they strive for the best possibly outcome for individuals at every given opportunity. We specialise in supporting individuals with complex needs, and are also able to offer support to individuals who require support to look after their children.
