The Oldham Care Response Team /Intermediate Care Service is a dedicated multi-disciplinary team of health and social care professionals whose aim is to provide high quality, holistic and individualised patient focused care in an appropriate setting based on comprehensive assessments of patient need.
Our aim is to avoid unnecessary hospital admission and facilitate early hospital dis”Charge” .
Referrals can be made by GP, Self referral, Social care professional, Health professional.
